LANSING, Mich. — Sixty-four Sparrow Care Network (SCN) physicians and medical offices have been selected as a Patient-Centered Medical Home designated practice by Blue Cross® of Michigan for 2020.
The designees comprise Sparrow Medical Group, Memorial Healthcare practices and other independent physicians who are members of SCN, a physician-led clinically integrated network of over 600 physicians in Mid-Michigan. SCN emphasizes quality and efficient care and ensures patients receive the best possible care at the lowest possible cost.
A Patient-Centered Medical Home emphasizes a holistic team approach that addresses chronic conditions and prevention. It stresses the partnership between patients and their personal physicians and the coordination of patient care with any physician or facility that the patient interacts with. PCMH-designated practices have made the most progress in implementing Patient-Centered Medical Home capabilities, and have strong performance on quality, use, and efficiency measures.
Under the system, providers work together with patients to improve their care according to their personal health goals.
Data from 2020 show in comparison to non-PCMH practices, Blue Cross PCMH-designated practices had a 35.5 percent lower rate of adult readmissions to the hospital, a 29 percent lower rate of pediatric Emergency Department visits, and much lower rates of adult inpatient and ED stays.
